Job Description
POSITION SUMMARY:
The primary goal of the NVIH Community Health and Outreach Worker (CH/ORW) position is to identify individuals, families and communities' strengths and needs. The CH/ORW supports individuals, families and communities in developing and meeting health enhancement goals. This is done in part by presenting resource options, providing health education and supporting decisions that promote well-being. The CH/ORW is a health advocate for the Native American in coordination with other related departments of Northern Valley Indian Health and community agencies. CH/ORW's receive training in a wide variety of health topics to be able to provide basic health teaching and coaching. Their services are individual, family, and community focused and related to all ages. This is an entry level position to which incumbents are expected to progress in their learning and training to become a Community Health and Outreach Representative (CH/ORR).
ESSENTIAL JOB FUNCTIONS:
Essential functions, as defined below may include any of the following knowledge, skills, and abilities. The list that follows is not intended as a comprehensive list, but rather to provide a representative summary of the major duties and responsibilities. Incumbent(s) will be required to perform a variety of duties listed, including but not limited to:
  • Outreach to the Native American population in an assigned service area to educate them of Northern Valley Indian Health's services.
  • Participate in the overall Community Health and Outreach Program under the direction and guidance of the Community Health and Outreach Director(s).
  • Provide care coordination services to the service area by:
  1. - Assisting patients in identifying service needs and appropriate resources.
  2. - Making appropriate in-house or out-of-agency referrals.
  3. - Assisting eligible patients in securing available financial assistance for health care related expenses.
  • Providing health education, coaching, support, and services focused on general health maintenance and promotion.
  • Support department with networking activity by proactively partnering with CH/ORR's to participate in community activities or events.
  • Provide transportation and/or delivery of emergency food in situations of urgent need per NVIH procedures.
  • Participate and support vaccine clinics on site and off site with a variety of duties including but not limited to administering vaccines.
  • Other duties as assigned.
Experience and Skills
REQUIRED QUALIFICATIONS:
  • High School Diploma or equivalency / GED.
  • Prior experience in a health or service related field preferred.
  • Medical Assistant preferred.
  • Effective written and verbal communication skills and ability to speak publicly, such as conducting group trainings.
  • Ability to maintain effective working relationships.
  • Proven ability to engage in culturally sensitive manner with our Native communities
  • Able to effectively work independently with minimal supervision.
  • Able to travel for training and maintain a valid California driver's license and insurability with the agency auto-policies along with maintaining reliable transportation.
  • Able and willing to learn and utilize health information and skills.
  • Basic computer skills required; internet browsing/researching, typing, email, etc.
  • Maintain current CPR certification.
  • Child Passenger Safety Seat Tech certification preferred.
